Line of Communication:
Any concern pertaining to PHC or Anatomy and Physiology subjects should first be brought to the attention of the faculty concerned. If the need or problem can not be resolved by the faculty alone, then the issue can be brought to the attention of the cluster coordinator, then the level coordinator or higher depending on the manageability of the concern. The student must always follow this line of communication.
Attendance:
1. Students are required to attend the classroom and the RLE activities (Related Learning Experiences) punctually and regularly.
2. A student who will incur absences equal to 20% of the total number of hours allotted for the course, whether excused or unexcused will be considered to have failed due to absences and get a grade of 5.0 (per CHED rules and regulations)
3. Tardiness for three consecutive class days shall be considered one unexcused absence.
4. A student with unexcused absence during an RLE day gets a failing grade for that day. For excused absence, one RLE day is equivalent to one day make-up.
5. Official notice of absence/tardiness MUST be secured from the AHSE office and accomplished properly. It must be submitted with a explanation letter signed by the parent or guardian to the faculty concerned. A photocopy of any valid ID of the signatory, with picture and signature, must be attached with the letter.
6. A student who fails to submit an official notice of absence/tardiness upon reporting for class will still be considered absent until an official excuse slip is submitted.
7. Absence due to illness should be reported to the faculty-in-charge and corresponding letter of explanation and medical certificate from a licensed physician must be submitted upon reporting to class or RLE. The letter should be duly noted by your parent or guardian and submitted together with a photocopy of his/her valid identification card. Letter must be properly addressed, (with complete name of the faculty or clinical instructor under whom the absence was incurred), written on short bond paper and with signature over printed name. Indicate the cluster, section and group.
8. For absence due to death of an immediate relative, follow guideline number 5. A photocopy of the death certificate must be submitted together with letter of explanation.
9. In case of absence during unit or final examination, a student may be given a special exam if it was due to illness or circumstances under force majure. In such event, absence classified as excused will be treated as a regular exam. For any unexcused absence during examination, the highest possible grade for any exam will be equivalent to a grade of 3.0 or 60 %.
10. No special quiz will be given to either excused or unexcused absence.
11. The official dropping period is one week before the midterms. Dropping from any class beyond this period is not allowed and the student gets a failing grade at the end of the semester.

2. No accessory is allowed. Only plain wrist watch with second hand can be worn (not digital, not oversized, no fancy, loud, & colorful designs).
3. All body piercing, tattoo and colored contact lenses are not allowed.
4. Hair requirements:
a. male students: Hair must not cover the ears nor reach/cover nape.
b. female students: Hair should be put up in a bun so that no hair will touch the collar of the uniform. Use only black clips, hair net or hair pins. Ribbon is not. allowed.
c. Hair dyes, highlights, streaks or tints are not allowed
5. Finger nails should be kept short and clean. No colored nail polish.
6. Beards and mustache should be shaved.
7. For violation, such as incomplete uniform or any misconduct:
Ø For any offense – corresponding demerits will be given by the faculty-in-charge
Ø Student’s ID card will be confiscated and turned over to the AHSE office with notation regarding the offense on a designated form for recording. ID card may be retrieved by the student from the AHSE office.
Grading System
1. The grading system for any course (didactic and RLE) shall be adopted from the standard grading system of the university which is zero based. (Passing rate is 60% of the total items for any given exam)
2. RLE Evaluation Tools are made transparent to students. The checklist by Kozier will be used to evaluate skills. Return demonstration of procedures will be given 60%, 30% will be from quizzes, long examinations, and 10% for attendance and attitude.
3. Any change in the grading criteria/system shall be communicated to the students prior to the actual conduct of classroom or RLE activities.
4. A student may consult with the faculty-in-charge for his/her grade during consultation hours or upon appointment.
Requesting Room/Laboratory/Equipment
1. Student should accomplish appropriate request slip at the IN office three (3) days before the date the materials requested are needed.
2. Adviser/faculty in charge is required to countersign the slip prior to submission. This makes the adviser/faculty responsible for the request.
3. For AVR use, application should be filed one (1) week before actual use.
Return Slip: Please print this portion after reading the guidelines. Return slip must be signed by you and your parent/guardian. Submit this to your faculty on your first RLE Day.
